Initial Licensure in Arizona

Psychologist and Behavior Analyst Licensure

The Board issues licenses to practice psychology as well as behavior analysis in the State of Arizona. The standard psychologist and behavior analyst licensure applications may be downloaded from PSY APPLICATIONS and BA APPLICATIONS pages on this website. Go to FORMS/APPLICATIONS in the menu tab, then click on the application page for your profession. Applications and related forms are linked at the top of the page. You may need to scroll down for more detailed information regarding the type of application appropriate for your professional history. 

A psychologist may apply for licensure by credential with one of the following designations:

  • American Board of Professional Psychology (ABPP)
  • Certificate of Professional Qualifications (CPQ)
  • National Register of Health Service Providers in Psychology (Note: Must demonstrate passing score on EPPP and five years of independent practice)

A psychologist or behavior analyst may apply for licensure by universal recognition if specific qualifications are met.

ONLINE APPLICATION PORTAL

Use the portal to complete the online renewal application or to pay your license issuance or renewal fee. 

We do not have an online application for new applicants. New Psychologist and Behavior Analyst applicants must complete a PDF application. Please go to the FORMS tab in the menu, and then go to the application page appropriate to your profession (PSY or BA) for further information and to download forms, including the Universal Recognition applications.

Pay Application Fees
by Credit or Debit Card

Staff cannot process credit/debit cards. If you wish to pay your application fee online with a card, please submit your application first (email is acceptable) and let Board staff know you want to pay the fee with a card. Staff must enter your application in the database before the application fee can be paid via the online application portal. Otherwise, initial application fees may be paid by check or money order via mail or delivery.

 

INITIAL ISSUANCE OF LICENSE

(Does not apply to Renewals):  

Approved Applicants are required to pay the pro-rated license fee to have their license issued. This may be paid online by card, or by mail if using a check or money order.  If paying by card using the online portal, please email the Board office first. The initial license fee will appear in the applicant's secure shopping cart only after staff assistance.
