Reopen EPPP Only Application for Approval for Licensure
For EPPP Only Applicants who have passed the EPPP
- Once you have passed the EPPP, your application will be administratively closed.
- The application may be reopened within 36 months for the purpose of having additional supervised professional experience (SPE) hours required under A.R.S. 32-2071(D) approved for licensure.
- Written Reopen Request: When you ready, you may email a written request to reopen your EPPP Only application for the purpose of approval of supervised professional experience (SPE) for licensure. There is no fee to the Arizona Board to reopen the application. Please email the request to the Board's Licensing Specialist at [email protected].
- Fingerprint Clearance Card: If you did not submit a copy of the front and back of your fingerprint clearance (FP) card with your EPPP Only application, it must be submitted with the reopen request. Statutes requiring a Department of Public Safety (DPS) FP card were enacted by the Arizona State Legislature in 2021. Changes to the statute language occurred in 2024, and federal approval of the statutes was granted to this Board in early 2025. Therefore, a copy of the front and back of your FP card OR proof of submitted application for the FP card to DPS is required. - Supervised Experience Verification: Verification of the SPE you are applying towards licensure must be submitted by your supervisor directly to the Board's office via email at [email protected]. (For preinternships, the DCT is the appropriate verifier.)
- Amount of hours required: Per A.R.S. § 32-2071(D), 3,000 hours of SPE are required for licensure. These may be any combination of preinternship, additional internship or postdoc hours. The internship verification would have already been approved with the EPPP Only application. Therefore, for example, if an applicant's internship verification was for 1,500 hours, an additional 1,500 hours of SPE are required to meet the 3,000 hours requirement. If the internship verification was for 2,000 hours, only 1,000 additional hours of SPE are required. See the Board's Forms page to download the appropriate SPE verification form. Follow the instructions on the Forms page and on the verification itself. The requirements and the verification form for preinternship SPE hours are different than for a postdoc SPE.
- NPDB Self-Query: You will need to submit a new National Practitioner Data Bank self-query processed within 30 days. NPDB self-queries are valid for 6 months. Email the official PDF downloaded from https://npdb.hrsa.gov to the Board's office. The official copy has NPDB's verifiable digital signature. Copies are not acceptable.
- References: If the completed reference forms in your file are dated more than 12 months prior to the request to reopen the application, new references will be needed. References are valid for 12 months and must be within that time frame at the time the Board reviews the reopened application.
- Substantive Review: Once the written request, FP card, new NPDB, new references if applicable, and verification of SPE are received, the reopened application is administratively complete and will be placed on the next available Application Review Committee agenda for substantive review. The reopened application will go through the same approval process as the original EPPP Only application. If the Committee recommends approval, the application will be placed on the Board's Consent Agenda. If approved by the Board, the applicant's license will be issued.
