Supervised Temporary License Search

To verify a supervised temporary license, this Directory serves as a primary source verification of the credential issued by the State of Arizona Board of Psychologist Examiners. This is the only verification of licensure provided.
Emergency Temporary License Search
Emergency temporary licenses for Psychologist and Behavior Analyst independent practice were issued between 04.09.2020 through 09.30.2022 in response to the state of emergency due to COVID-19. The Board ceased accepting applications for this temporary license as of 10.01.2022. Under A.R.S. § 32-3124(D), a temporary license is no longer valid upon issuance or denial of a full, independent Arizona license. All active emergency temporary licenses will expire on 01.01.2023 per Senate Bill 1309 signed by Governor Doug Ducey on 03.25.2022. For a complete list of emergency temporary licenses issued, please download the directory, below. The below PDFs are identical in content; however, one is sorted by last name of the licensee, and the other is sorted by license number.

Address Opt-Out
Pursuant to A.R.S. § 32-3226, licensees who do not have a business address may OPT OUT of the requirement to make their home address public (A.R.S. § 32-3801). Licensees who opt out must provide a phone number or email address to the Board which can be disclosed to a patient or client who is seeking a copy of their medical records. If you fall into this category and wish to opt out, please email the Board office with a written request to opt out per A.R.S. § 32-3226.
For Credentialers - Primary Source Verification Statement:
The Arizona Board of Psychologist Examiners (Board) has three applications for licensure for psychologists and two applications for behavior analysts.
For the standard Psychologist Application for Exam and/or Licensure and the standard Behavior Analyst Application for Licensure, the Board conducts primary source verification of an applicant’s graduate education, training and passage of the national exam before issuing a license to practice psychology or behavior analysis in Arizona.
For standard psychology applications, some verification(s) of training may be waived based on number of years in independent practice:
-
For 10 years or more independent practice, the applicant may waive verification of additional 1,500 hours supervised experience.
-
For 20 years or more independent practice, the applicant may waive verification of all supervised experience/training.
For the Psychologist Application for Licensure by Credential, the credential (CPQ, ABPP or National Register/HSP) is primary source verified. Education and training are not verified by the Board for licensure by credential applications. For applications with the National Register credential, passage of the national exam is also primary source verified.
For Psychologist or Behavior Analyst Applications for Licensure by Universal Recognition, Arizona statutes and rules do not permit the Board to verify education and training.
A National Practitioner Data Bank self-query is required for all psychology applicants.
For all applications for licensure, verification of licensure/certification and disciplinary history from every jurisdiction in the U.S. and Canada in which a psychologist or behavior analyst holds or has held a license/certification is obtained directly from the licensing boards for each applicant, as applicable.
