Behavior Analyst Application for Licensure

Behavior Analyst Application for Licensure by Universal Recognition
DO NOT use this application if you DO NOT have a license in another U.S. State or Territory. BCBA certification is NOT a license. You must have a license in another U.S. State or Territory for at least ONE YEAR to be qualified to apply for licensure by Universal Recognition. Credit/Debit Card Payments for Application Fees
Staff cannot process credit/debit cards. If you wish to pay your application fee online with a credit card, please submit your application first (email is acceptable) and let Board staff know you want to pay the fee via credit/debit card. Staff must enter your application in the system before the application fee can be paid via the online application portal. Otherwise, initial application fees may be paid by check or money order via mail or delivery.

THINGS TO NOTE ABOUT THE APPLICATION PROCESS
Time frame from receipt of application to approval averages 3-4 months and is entirely dependent on how quickly verifications are received. For example, if you wish to be licensed in July, please send your application in 3-4 months ahead.
Applicants may request verifications or other documentation sent to the Board in advance of their application. Verifications, transcripts, and any other documents received in the Board's office are retained for 12 months and are matched to the application once the application is received. All documents and verifications may be received by via upload to the online BA Documents Submissions portal.) License verifications and official transcripts may be sent by the verifier to [email protected].
Supervised Experience hours cannot be verified by the BACB. First, the BACB does not disclose this information to third parties. Second, the BACB is not the primary source for this documentation. If you wish to use the same hours for licensure that you used for your BCBA certification, you may have your supervisor submit a copy of the BACB Final Experience Verification form the supervisor signed for your BCBA certification. Otherwise, the supervisor may complete the Arizona verification form, "Supervised Experience or Independent Fieldwork Verification", which can be downloaded from this page. If your supervisor is no longer available, you must submit documentation of your attempts to contact your supervisor in order for the Arizona Board to accept the BACB Final Experience Verification Form from you.
Please allow up to three (3) weeks for staff to complete the initial intake of your application and email you a a notice of deficiency.
We do not have an online application at this time. All applicants must submit a form-fillable PDF application by email or mail. Use Adobe Acrobat or Adobe Reader for best experience when filling out an application. All other programs and apps change the functions of the document.
A.A.C. R4-26-201 requires that an application be placed on an agenda if it is administratively complete 18 days prior to the next scheduled board meeting. This also means that an application that is not administratively complete cannot be placed on an agenda. "Administrative completeness" means that all required verifications and documentation have been received and processed by Board staff. Please be aware that staff has up to 100 applications open in various stages of completeness at any given time, and applications and documentation are processed in the order they are received. If any item has not been received, you will receive a notice of deficiency and your application will be placed on the next available agenda AFTER the missing documentation is received.
It is the applicant's responsibility to request an update after the initial notice of deficiency has been emailed to the applicant by Board staff. For updates regarding your application, please email the licensing specialist at [email protected]. It is up to the applicant to keep track of when documentation has been sent, and request an update regarding the Board's receipt of that documentation.
Behavior Analyst Application for Licensure
Behavior Analysts are required to be licensed by the Board of Psychologist Examiners to practice independently in Arizona. Instructions and requirements are on the first page of the application packet. Current Version has 11/2019 Revised Date. We do not have an initial online application. Please see Arizona Revised Statutes (A.R.S.) § 32-2091.02, A.R.S. § 32-2091.03 and Arizona Administrative Code (A.A.C.) R4-26-203 through R4-26-406 for requirements. PLEASE NOTE, as of August 1, 2019, verification that an applicant passed the BACB's exam will be independently obtained by Board staff.
Supervised Work Experience or Independent Fieldwork Verification
(Make as many copies as needed) If you are applying the same hours towards licensure that you used for BCBA certification, your supervisor may submit your BACB Final Experience Verification forms instead of this verification form. If you are applying different supervised hours, use this verification form.
R4-26-403(C)(2) requires that verifications be sent directly from your supervisor to the Board's office. Verifications included with your application will not be accepted.

On August 27, 2019, the Arizona Board of Psychologist Examiners will begin accepting applications for licensure by universal recognition. WHO IS ELIGIBLE TO APPLY BY UNIVERAL RECOGNITION?
The universal recognition policy (A.R.S. § 32-4302) allows Arizona residents to use an out-of-state professional or occupational license to qualify for an Arizona license to work. To qualify, an applicant must:
- Prove residence in Arizona (See the Board's Substantive Policy Statement regarding this). You must be a resident of Arizona to qualify for licensure by Universal Recognition.
- Be currently licensed or certified for at least one year in another U.S. state or territory in the discipline applied for and at the same level of practice as recognized in Arizona. BCBA certification is NOT a license.
- Be in good standing in all states where currently or previously licensed or certified. Verification is required directly from the licensing board to the Arizona Board of Psychologist Examiners for all licenses or certifications ever held regardless of the status of the license or certification. Please contact your licensing board for their instructions to request to have verification sent to the Arizona Board of Psychologist Examiners. Verifications from other states may be emailed from the respective boards to [email protected].
- Have met all applicable education, work, exam, and/or clinical supervision requirements in the other state where originally licensed or certified.
- Pay the $350 application fee either by check or money order to the Arizona Board of Psychologist Examiners or by credit/debit card via the online application portal. YOU MUST FIRST SUBMIT YOUR APPLICATION AND LET BOARD STAFF KNOW YOU WISH TO PAY BY CREDIT/DEBIT CARD. The application must be entered into the system before the fee can be paid via the online portal. You may submit your application by email to [email protected].
Application for Fee Waiver
Arizona Revised Statutes § 41-1080.01 provides for a waiver of “any fee charged for an initial license for any individual or applicant whose family income does not exceed 200% of the Federal Poverty Guidelines if the individual is applying for that specific license in this state for the first time.” If you believe that you qualify for the waiver, please use the application below and submit with your initial license application.
